EPC/ Chronic Disease Management plans 

All Medicare card holders are eligible for rebates on service provided under the EPC/ Chronic Disease Management plans. Client will need to pay for a gap fee.

Department of Veteran’s Affairs

Leslie provide bulk bill service for DVA gold card holder. A gap fee will not be charged.
