Please contact your health funds for more information.</p> <p class="font_7">&nbsp;</p> <p class="font_7">Rebate is limited to face to face consultation only.&nbsp;</p> <p class="font_7"><span class="wixGuard">​</span></p> <p class="font_7">You receive a rebate from Medicare only if you have an Chronic Disease Management Care Plan from your GP and your eligibility is determined by your GP. A GP referral letter and a copy of this care plan is required for your first consultation.&nbsp;</p>
